Application and Processing of VIP Card

To apply for a VIP Card, please fill out the registration form for the Medical Health Care Card, and provide one 1x1 ID photo. The annual fee for VIP Card is RMB2,000 per person, with one-year validity from the date of purchase. The VIP Card is non-transferable and for member’s exclusive use only.

Family Health Care Card fee is for every member of the family: RMB1,800 per person, RMB1,200 for any child 12 years or younger.

Benefits Enjoyed by VIP Cardholders

1. One-time over all physical check up: Examination by doctor, obstetric check up (for female), dental check up, electrocardiogram, chest X-ray, abdominal ultrasound, blood and urine tests, blood biochemical test (30 norms), cervical pap smear test (female).
   
   
2. One-time ultrasonic dental wash.
   
   
3. During the validity period of VIP Card, the holder enjoys 50% discount on clinical fees, and 20% discount on therapy, laboratory, X-ray, and Ultrasound check up.

Effective January 1, 2002, based on the original Health Care Card (RMB2,000 per person per annual), we have formally launched the Health Care Golden Card. It is aimed to facilitate you and your family members in receiving medical care in the Clinic. It also provides convenience to the business in health care of every employee.

Services Offered and Notice on Card Purchase for Health Care Golden Card Are as Follow:

1. Initial payment for the purchase of Golden Card by honored client is RMB8,000. The Golden Card shall be effective from the date of purchase.
   
   
2. When the Golden Card cardholder and his/her family member and friends come to the Clinic for treatment, fees can be debited against the Golden Card, with no head count limitation.
   
   • All Golden Card cardholders can enjoy 50% discount, and 20% discount on laboratory and therapy. Normality overall check up fee shall be discounted to RMB1,700 per person each time.
     
   • All Golden Card cardholders can enjoy health information services and series of promotional activities provided by the Clinic.
   
   
3. When the balance reaches RMB1000, a successive fee of not less than RMB5000 has to be paid. If all RMB8,000 in the card has been used, the cardholder cease to enjoy the privileges of being a Golden Card cardholder until successive fee is paid.
